In line with the WA government Vaccine mandate as of Monday 31st of January, all members 16 years of age and older will be required to have shown proof of vaccination to enter the premises at Cube43. 

 

While we respect everyone's individual choices and rights, we need to ensure we are following the state mandate & public health orders. Anyone not able to access the gym at this time, we have online options to suit you and help you with your fitness. 

 

You can provide evidence of your vaccination status in the form of:

• a copy of your COVID-19 Digital Certificate;

• a copy of your immunisation history statement; 

• proof of vaccination from a medical practitioner.

• a medical exemption from a medical practitioner.

Once your evidence received your member profile on Zen Planner will be updated to show that your vaccination status has been verified.  We will just keep note your proof has been viewed on your profile and all other proof will be stored securely.

Your documentary evidence will only be:

• viewed by staff who have a specific need to access it to do their job; and

• used for the sole purpose of verifying your vaccination status in relation to your access to and use of Cube43 Fitness.

Other than as specified in this Collection Statement we will not disclose your evidence or vaccination status information to any third party unless required or authorised to do so by law or a court or tribunal order. However, we may share your information with relevant regulatory bodies if they wish to check our compliance with the PHOs so they can do so.

We will take reasonable steps to secure this information from unauthorised access, use or disclosure and from loss.
